MAPS Charities works with scores of agencies throughout Los Angeles County. Their Professional Senior Advocates (PSAs), include social workers, senior care managers, physicians and discharge planners.

If you are senior or know of a senior in need who might benefit from a MAPS Charities grant, please contact your local senior center to refer you to a Professional Senior Advocate.

My Place Health Using the PACE (Program of All-Inclusive Care for the Elderly) model, My Place Health provides primary care, integrated health plan coverage, personalized social engagement, and customized services. To qualify for the PACE program, adults must be age 55 or older, eligible for nursing home care, and able to live safely in the community with help from PACE.
